We understand how important your care is and we do not want financial hardship to stand in the way of you or your child's mental health goals. We have a sliding fee scale for families who make under 70K per year who demonstrate hardship. In advance of completing this form, please have your insurance deductible information ready along with your rate of reimbursement for out of network services.
